Host an Atlas CareMap workshop in your community

The Atlas CareMap Community Workshop Toolkit is a resource for anyone who would like to run an Atlas CareMap workshop in their neighbourhood or community, either online or in-person. A Care Map is a simple, visual diagram of the people involved in providing care to a caregiver and care recipient. A Care Map allows a person to visualize and easily understand the many different connections between the people involved in caring for someone.

The toolkit will help increase awareness, understanding, and use of the care map tool and provide information on how it can help caregivers and care recipients. This toolkit contains everything that will be needed to run a care map workshop including a coordinator guide, templates, presentations, posters, requests for funding and more.

Resources included in this toolkit:

  • Coordinator guide
  • Quick start guide
  • Atlas CareMap fact sheet
  • PowerPoint presentation
  • PowerPoint presentation references
  • Discussion activity
  • Poster template
  • Agenda template
  • Attendance sheet
  • Checklist
  • Confirmation email template
  • Funding request template
  • Invitation template
