The Social Connection Palliative Care – Module is a free educational resource offered as part of the Care Connections Program. It helps health care professionals understand the critical role of social connections in patient health outcomes. The module explores the use of the Atlas CareMap as a tool to increase patients’ social connections. After completing the module, health care professionals will have a better understanding of what an Atlas CareMap is, the impact of using a map in a medical and non-medical setting, and how to integrate its use into daily practice.
