Who is it for?

LEAP Heart is ideal for any health care professional, such as physicians, nurses, pharmacists, social workers, and more, who provides care to patients with advanced heart diseases.

This course promotes teamwork, supports interprofessional collaboration, and fosters connections among health care providers across cardiology settings (e.g., cardiologists, cardiac nurses, coronary care unit staff, social workers) who work together to support patients with advanced heart disease.

Learning outcomes

Upon completion of LEAP Heart, learners should be able to:

  • Define palliative care and identify patients with advanced heart diseases who would benefit from it.
  • Identify personal biases and misunderstandings related to palliative and end-of-life care.
  • Provide person-centred, compassionate care tailored to the needs of patients with advanced heart diseases.
  • Undertake Advance Care Planning (ACP) and goals of care discussions specific to cardiology patients.
  • Identify and assess symptoms and needs of patients with advanced heart diseases.
  • Address psychological, social, and spiritual needs of patients and their families.
  • Ensure smooth transitions of care between different health care settings.
  • Engage in and undertake essential conversations with patients and their families.
  • Collaborate effectively with interprofessional teams to provide comprehensive palliative care.
  • Implement quality improvement initiatives in palliative care practices.

  • Self-paced learning: Complete 17 interactive, self-learning modules at your own pace (approximately 8 hours of work).
  • Workshop: Participate in 6 hours of live webinars led by LEAP facilitators where learners will work through cases and discuss learnings from modules

Accreditation

This 2 credit-per-hour Group Learning program has been certified by the College of Family Physicians of Canada for up to 29.0 Mainpro+ credits.

This event is an Accredited Group Learning Activity (Section 1) as defined by the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada, and approved by the University of Ottawa’s Office of Continuing Professional Development.
