After completing LEAP Hospital, I added a reminder in the Electronic Medical Record (EMR) to identify patients who might benefit from a palliative approach. This initiative has helped me recognize several patients with end-stage but stable COPD, allowing me to initiate important conversations about end-of-life care and symptom management—discussions I might not have considered at this stage before.
