Who is it for?

LEAP Mini is ideal for any health care professionals such as physicians, nurses, pharmacists, social workers and more, whose primary focus of work is not palliative care but who provides care for patients with life-threatening and progressive life-limiting illnesses.

LEAP Mini promotes teamwork, supports interprofessional collaboration, and fosters connections among providers in different agencies who work together (e.g., community-based family physicians, home care nurses, and community pharmacists).

  • Promote and undertake Advance Care Planning discussions.
  • Assess and manage pain; delirium; dyspnea; and nausea, nutrition, and hydration.
  • Develop plans to address spiritual, religious, or psychosocial needs.

LEAP Mini was a wonderful learning experience for me. I have recently begun working in our hospice unit in our facility and it has answered many of the questions that I have had. This course has given me many tips as to how to handle different circumstances that occur on a daily basis. It has provided me with tools that will allow me to become a better care provider to people receiving palliative care.

Excellent course! The facilitators were amazing, their wealth of information together was great, and they invited a relaxed learning/sharing environment which allowed for great learning.
Nurse (RPN)

Even throughout my family medicine blocks, ER, ICU, etc., you see people needing palliative care. Since completing the LEAP Mini course, I’ve done palliative consults, explained palliative care as an option to patients, and have become more comfortable managing opioids in all settings.
